A glaring omission in your bibliography is the name of Greg Sandow, the influential Village Voice critic who succeeded Tom Johnson and preceded Kyle Gann.

I'm well aware of Sandow's work at a crucial time in the history. Unfortunately, his work has not been collected in book form yet. It really should be, and then I'd definitley add him to the bibliography. As it is, Voice articles from that era are hard to find.
